Clinical aspects of pleural fluid pH
نویسنده
چکیده
Pleural fluid pH is of greatest value for the assessment of patients whose pleural effusion is due to respiratory infection (pneumonia), but that is not the limit of the test’s utility. In addition to the clinical value of measuring pleural fluid pH, the article includes consideration of measuring method and sample collection; it begins, however, with a brief discussion of some relevant anatomy and physiology.

Pleural fluid pH as a predictor of pleurodesis failure: analysis of primary data.
STUDY OBJECTIVES To determine the predictive accuracy of pH for identifying patients with malignant pleural effusions who will fail pleurodesis. DESIGN Analysis of published and unpublished individual patient-level data retrieved from a MEDLINE search and correspondence with primary investigators. STUDY SELECTION Studies that reported pleural fluid pH values and outcomes of pleurodesis for ...
متن کاملDiagnostically significant variations in pleural fluid pH in loculated parapneumonic effusions.
STUDY OBJECTIVES Parapneumonic effusions are common, and measurement of pleural pH is one of the most useful measurements in assessing the need for tube drainage. Use of pleural pH assumes that a single measurement conveys a representative picture of pH throughout the effusion. Often effusions are multiloculated, and varying concentrations of nondiffusible acids such as lactic acid, if present ...
متن کاملPleural fluid chemical analysis in parapneumonic effusions. A meta-analysis.
Controversy exists regarding the clinical utility of pleural fluid pH, lactate dehydrogenase (LDH), and glucose for identifying complicated parapneumonic effusions that require drainage. In this report, we performed a meta-analysis of pertinent studies, using receiver operating characteristic (ROC) techniques, to assess the diagnostic accuracy of these tests, to determine appropriate decision t...
متن کاملPleural fluid analysis in malignant mesothelioma. Prognostic implications.
Pleural fluid characteristics of 26 patients diagnosed with malignant mesothelioma over an 18-year period were reviewed and compared with those of patients with effusions due to other malignancies. Survival from time of initial thoracentesis was directly correlated with pleural pH and decreased pleural fluid/serum glucose ratios but was not related to protein or LDH concentration. Decreased ple...
متن کاملThe diagnostic value of pleural fluid pH.
One hundred eighty-three patients had simultaneous blood and pleural fluid pH determinations. Thirty-six effusions were transudates, and 147 were exudates. In 46 effusions, the pleural fluid pH was less than 7.30; all 46 were exudates. A pleural fluid pH less than 7.30 was associated with the following six diagnoses: (1) empyema; (2) malignancy; (3) collagen vascular disease; (4) tuberculosis; ...
